They are very common (one in every five women) and are generally innocuous — thereby earning the epithet of being “womb stones that go to the tomb”! But don’t let that fool you because uterine fibroids could be the cause of a lot of pain in women because of the problems they could create. Heavy menstrual bleeding, menstrual pain, premature labour, preventing women getting pregnant (infertility) are all conditions caused by these non-cancerous tumours of muscle mass in the uterus (womb).

Yes, it’s uterine fibroids we are talking about and these have the potential to grow haphazardly to large sizes and literally start throwing their weight around. They could press on the bladder, large veins (blood vessels) or on the tube connecting the kidney to the urinary bladder (the ureter) causing back pressure and kidney damage. But they very rarely turn malignant.

To treat or not to treat? So, what’s the treatment for fibroids? The answer is “Let sleeping dogs lie!” Fibroids that are not up to mischief and are not troublesome — no matter what size and site — may be left alone and watched for growth. Every 6-12 months the size of these silent fibroids should be assessed by an ultrasound. Any rapid growth is a strong indication for removal. But a steady, slow growth is acceptable during the reproductive years of a woman. Grow they will because a woman’s wonderful hormones — the estrogens — nurture them! However, once a woman turns menopausal and is devoid of the abundance of hormones, the fibroids will gradually go to sleep and shrink in size. So, even a slowly growing fibroid in a menopausal woman is cause for a red alert and removal.

Myomectomy and hysterectomy Women who have completed their child bearing or are unlikely to be physiologically traumatised by the removal of a ‘feminine’ part of their body, may opt for a hysterectomy. The reason for choosing a hysterectomy is that any woman with the radiological or clinical presence of fibroids usually does have invisible, undetectable or tiny fibroids that can grow in the future and create trouble for the woman again. This is called recurrence and leads to the re-occurrence of the same or different problems and symptoms in women.

Pros and cons The pros of a myomectomy are that the uterus is preserved. And if the problem caused by the removal of fibroids is also dealt with, the woman can continue to have her periods and may not be psychologically traumatised or feel less feminine. However, with a myomectomy, fibroids may reoccur. Not all the fibroids need to be removed and some may even grow again to create future problems.
